銀屑病俗稱(chēng)“牛皮癬”，是常見(jiàn)的皮膚慢性炎癥性疾病。該病易于復(fù)發(fā)、無(wú)法根治、嚴(yán)重危害患者身心健康。臨床上包括anti-TNF、anti-IL-17A/IL-17R、anti-IL-23等在內(nèi)的抗體類(lèi)藥物靶向免疫系統(tǒng)及相關(guān)因子治療銀屑病有良好預(yù)期和效果，但副作用大，停藥后疾病易反彈。因此，深入研究銀屑病的發(fā)病機(jī)制、治療新靶點(diǎn)和新策略勢(shì)在必行。近日，上海交通大學(xué)基礎(chǔ)醫(yī)學(xué)院王宏林教授團(tuán)隊(duì)在國(guó)際免疫學(xué)頂級(jí)期刊Immunity在線發(fā)表了題目為“Excessive Polyamine Generation in Keratinocytes Promotes Self-RNA Sensing by Dendritic Cells in Psoriasis”的研究論文，該研究發(fā)現(xiàn)白細(xì)胞介素17 (IL-17)下調(diào)了銀屑病角質(zhì)形成細(xì)胞中的蛋白磷酸酶6(PP6)，導(dǎo)致磷酸化和轉(zhuǎn)錄因子C/EBP-β的激活以及隨后產(chǎn)生的精氨酸酶-1。角質(zhì)形成細(xì)胞中缺乏PP6的小鼠易患牛皮癬樣皮膚炎癥。
該研究發(fā)現(xiàn)，PP6缺失的表皮細(xì)胞精氨酸酶(Arg1)上調(diào)導(dǎo)致尿素循環(huán)下游代謝物——多胺(Polyamines)異常地在皮膚部位累積。Polyamines攜同表皮細(xì)胞釋放的自身核酸與具有RNA結(jié)合能力的短肽形成復(fù)合物，而來(lái)自表皮細(xì)胞的“多胺+自身核酸+短肽”復(fù)合物形成“自身抗原”進(jìn)入樹(shù)突狀細(xì)胞(DCs)后強(qiáng)烈激活內(nèi)體中的Toll樣受體-7，并介導(dǎo)DCs釋放多種促炎因子。皮膚局部注射精氨酸酶抑制劑顯著改善小鼠與非人靈長(zhǎng)類(lèi)動(dòng)物模型的類(lèi)銀屑病皮膚炎癥。該研究首次揭示，組織特異性細(xì)胞的代謝重編程所引起的代謝物異常累積是銀屑病等慢性炎癥性疾病發(fā)生發(fā)展的重要“幫兇”，表皮細(xì)胞的“多胺+自身核酸+短肽”復(fù)合物是激活皮膚免疫反應(yīng)的“自身抗原”，而靶向調(diào)節(jié)表皮細(xì)胞精氨酸代謝通路的異常有望成為治療銀屑病的新策略。（文章內(nèi)容參考https://www.cell.com/immunity/fulltext/S1074-7613(20)30234-X,https://www.shsmu.edu.cn/cbms/info/1077/3856.htm）
