近期，西安交通大學(xué)第一附屬醫(yī)院陳超教授、侯鵬教授及其團(tuán)隊(duì)合作在《Aggregate》（IF=18.8）上發(fā)表題為“Smart multi-functional aggregates reoxygenate tumor microenvironment through a two-pronged strategy to revitalize cancer immunotherapy”（智能多功能聚合體通過雙管齊下的策略使腫瘤微環(huán)境重新充氧，以促進(jìn)癌癥免疫治療）的研究。
PD-1/PD-L1抑制劑已成為晚期實(shí)體瘤的標(biāo)準(zhǔn)治療方法；然而，總體反應(yīng)率低與系統(tǒng)性副作用等挑戰(zhàn)阻礙了其實(shí)施。缺氧促使腫瘤微環(huán)境的重塑是免疫療法失敗的主要原因。盡管目前已經(jīng)報(bào)道了一些緩解缺氧的策略，但它們的個(gè)體局限性限制了進(jìn)一步的改進(jìn)。本文中，研究團(tuán)隊(duì)提出了一種新穎的雙管齊下的策略，通過同時(shí)采用阿托伐醌（ATO，抑制氧消耗）和氧合血紅蛋白（HbO2，直接補(bǔ)充氧氣）在名為 NPs-aPD-1/HbO2/ATO的多功能聚合體中有效地解決缺氧問題。除了用這兩種成分消除缺氧外，這種智能聚合體還包括白蛋白和 ROS 響應(yīng)交聯(lián)劑作為控釋支架，以及用于免疫治療的 PD-1 抗體 (aPD-1)。
有趣的是，NPs-aPD-1/HbO2/ATO 在體內(nèi)表現(xiàn)出特殊的腫瘤靶向性，在腫瘤中的積累量比在肝臟中高約 4.2 倍。因此，這種聚合體不僅有效緩解缺氧并顯著輔助aPD-1免疫治療，而且同時(shí)解決了與每個(gè)成分單獨(dú)給藥相關(guān)的靶向和全身毒性問題。這項(xiàng)研究對(duì)藥物靶向遞送、解決腫瘤缺氧和推進(jìn)免疫治療具有重要的意義，為推進(jìn)癌癥治療策略提供了有價(jià)值的見解。
